Cost overrun in construction projects is a common problem that can disrupt the success and sustainability of the project. This study aims to identify the dominant factors causing cost overrun and formulate mitigation strategies for the construction project of the Waron Maternity Hospital, Surabaya. The method used was a survey with purposive sampling of 15 respondents with at least two years of experience, using a Likert-scale questionnaire. Data were analyzed using the Relative Importance Index (RII) and Cronbach's Alpha reliability tests. The results indicate that dominant factors include material price increases, inaccurate unforeseen cost estimates, inadequate cost control, and inadequate coordination between the project owner and contractor. Proposed mitigation strategies include implementing unit price contracts with price escalation clauses, conducting pre-project material market studies, improving communication, workforce training, and conducting regular project evaluations. These findings are expected to serve as a reference for cost control and risk management in hospital construction projects in Surabaya.
